چکیده

BACKGROUND Chronic obstructive lung disease (COPD) is a frequent co-morbidity in patients hospitalised with community-acquired pneumonia (CAP). In recent retrospective studies, higher mortality in patients with CAP and COPD was found. OBJECTIVES The aim of the study was to determine the 30-day mortality and to evaluate the differences in CAP severity scoring in hospitalised patients with COPD. METHODS A subanalysis of a randomized clinical trial was performed. RESULTS A total of 262 patients with CAP were included. Ninety-five (36.3%) patients had COPD. A total of 28 (10.7%) patients died within 30 days. No differences between patients with and without COPD in 30-day mortality were observed [8 (8.4%) vs. 20 (12.0%), p = 0.37]. In the Pneumonia Severity Index (PSI), significant differences in age, gender and heart rate between patients with and without COPD were observed. Patients with COPD were stratified in higher PSI classes. In the CURB-65 score, age >or=65 years was significantly higher in patients with COPD [72 (75.8%) vs. 88 (52.7%), p = <0.01]. In a multivariate analysis, only the need for intensive care unit admission and high serum glucose were predictors of mortality [OR 32.50 (95% CI 6.87-153.75), p < 0.01; OR 7.34 (95% CI 1.19-45.4), p = 0.03]. CONCLUSIONS Mortality was not increased in patients with COPD hospitalised with CAP. Severity scores are influenced by age and gender. Further studies evaluating CAP in patients with COPD are needed to explain these findings.

Hyperglycemia and Red Cell Distribution Width for Prediction of Mortality in Preschool Children with Community Acquired Pneumonia (CAP)

Background Community acquired pneumonia (CAP) is a major infectious cause of mortality in preschool children especially in developing countries. Red Cell Distribution Width (RDW) has been associated with poor outcomes of CAP. We aimed to determine whether admission stress hyperglycemia and RDW can predict mortality in preschool children with CAP for early identification of patients at risk of ...

COPD is associated with increased mortality in patients with community-acquired pneumonia.

Patients with chronic obstructive pulmonary disease (COPD) who develop community-acquired pneumonia (CAP) may experience worse clinical outcomes. However, COPD is not included as a distinct diagnosis in validated instruments that predict mortality in patients with CAP. The aim of the present study was to evaluate the impact of COPD as a comorbid condition on 30- and 90-day mortality in CAP pati...

[Prognosis in patients with pneumonia and chronic obstructive pulmonary disease].

OBJECTIVE To study the incidence, severity, and mortality rates of pneumonia in a cohort of chronic obstructive pulmonary disease (COPD) patients monitored over 3 years. PATIENTS AND METHODS A total of 596 patients diagnosed with COPD according to spirometric criteria were included in the study. The variables assessed were mortality and severity according to the Pneumonia Severity Index (PSI)...

Severity and outcomes of hospitalised community-acquired pneumonia in COPD patients.

Chronic obstructive pulmonary disease (COPD) is a frequent comorbidity in patients with community-acquired pneumonia (CAP). We investigated the impact of COPD on outcomes of CAP patients. We prospectively studied the clinical presentation of 1,379 patients admitted with CAP during a 4-yr period. A comparative analysis of disease severity and course was performed between 212 patients with COPD, ...

Prediction of requirement for mechanical ventilation in community-acquired pneumonia with acute respiratory failure: a multicenter prospective study.

BACKGROUND Several severity scoring systems for predicting mortality are established in community-acquired pneumonia (CAP). OBJECTIVES The predictability of the aggravation such as requirement for mechanical ventilation in addition to mortality was examined in CAP patients with acute respiratory failure by using the age, dehydration, respiratory failure, orientation disturbance and blood pres...
